Étude préliminaire de prévalence des thromboses veineuses profondes et/ou superficielles des membres inférieurs en médecine vasculaire de ville chez les patients atteints de cancer.

Preliminary study of the prevalence of Deep or Superficial Venous Thrombosis of the limbs in Outpatient with cancer in private angiology practice. A French survey of the Société Française d’Angéiologie (SFA).

Résumé :

Le médecin vasculaire a un rôle majeur dans le diagnostic et la prise en charge de la thrombose veineuse profonde (TVP) et particulièrement chez le patient atteint de cancer, au sein de la chaîne de santé formée par les oncologues, les spécialistes de l’hémostase et les médecins traitants. La SFA a décidé de réaliser une étude sur la prévalence de l’association TVP et cancer chez des patients suivis en angéiologie de ville.
Nous avons aussi vérifié si leur prise en charge est en accord avec les recommandations françaises en particulier. Ces résultats préliminaires pourront servir de base pour la constitution d’un registre.
Méthodologie : Étude rétrospective menée chez 3 angéiologues de ville, basée sur les résultats des explorations vasculaires chez des patients avec cancer, actif ou non, et suspicion de TVP ou de TVS réalisées dans les 6 mois précédents.
Tous les écho-Doppler (ED) étaient réalisés avec la même méthodologie par duplex couleur, bilatéral, depuis les pieds jusqu’à la veine cave inférieure.
Résultats : Chez 430 patients adressés pour suspicion de TVP, le diagnostic a été confirmé chez 87 d’entre eux (20 %). Dix (soit 11 %) avaient un cancer : actif (4) et ancien (6).
Dans les deux tiers des cas, il s’agissait de thrombose veineuse proximale. Deux TVP étaient associées à des TVS, de siège multiple.
Une seule thrombose sur cathéter veineux central a été retrouvée dans cette période d’observation et nous l’avons incluse dans la discussion.
Le traitement anticoagulant initié par l’oncologue et/ou le médecin traitant et / ou l’angéiologue ,comportait une HBPM à dose curative 9 fois sur 10. Un relais par AVK avait été institué chez la moitié des patients. Les AVK ont été prolongés plus de 6 mois chez 7 patients, et 3 mois chez 2 autres.
Pour les 4 patients avec cancer actif, seulement 2 ont bénéficié d’un traitement de plus de 6 mois, un autre pendant 6 mois et le dernier, 3 mois.
Les patients de cette étude avec cancer non actif avaient tous des antécédents thrombo-emboliques et un traitement au long cours a été décidé chez eux.
Enfin, la compression médicale a été prescrite chez tous les patients avec TVP et/ou TVS.
Conclusion : Cette étude, préliminaire, montre que l’association TVP (et ou TVS) et cancer est retrouvée chez 10 %des patients adressés à l’angéiologue pour suspicion de maladie thrombo-embolique veineuse. Le traitement n’est pas toujours en accord avec les recommandations spécifiques de la thrombose chez le cancéreux.
La thromboprophylaxie reste sous-estimée dans les recommandations chez le cancéreux. Tout ceci encourage à la poursuite de cette étude en angéliologie de ville.

Summary :

Angiologists have a major role in the diagnosis and treatment of DVT, as they are in contact with oncologist, haemostasis specialists, and general practitioners.
The SFA decided to perform a study of patients recruited from private angiology practices, regarding prevalence of DVT and cancer.
We wanted to identify common risk factors in those patients and learn if management and treatment is in accordance with current recommandations and French guidelines. We present this preliminary study to show, the management of the patients, and to illustrate the importance of the problem in private practice.
Methodology: A retrospective study led by 3 private angiology physicians, based on medical records of patients who were sent for investigation of suspected DVT.
We included only patients who had DVT, and one case of superficial venous thrombosis (SVT), in relation with cancer, whether active or not. The examination was performed with the same methodology by colour flow duplex ultrasonography (bilateral from foot to vena cava).
Results: Our 3 private centres selected 430 files of patients for suspected DVT the diagnosis of DVT was confirmed in 87 patients (20%). Among those patients, 10 had previous or active cancer (11%), 6 a previous cancer, and 4 active cancer
2/3 of the thrombosis occured in proximal sites, one on a central venous catheter, and 2 associated with SVT, and in multiple locations.
The anticoagulant treatment was 9/10 a low molecular weight heparin (LMWH), with curative intent. Parenteral treatment with vitamin K antagonists (VKA) took place in a further half of patients.
The VKA was taken for more than 6 months in 7 patients, and 3 months in another 2 with lower limb thrombosis.
Among 4 patients who had active cancer, 2 had anticoagulation for more than 6 months, one for 6 months, and the last 3 months.
Patients who had an inactive or cured cancer, had all previous DVT, and had long term anticoagulation.
All patients were prescribed graduated compression stockings.
Conclusion: This preliminary study shows that the association of DVT and cancer is found in 10% of patients referred for suspected DVT by angiologists in private practice. The most frequently identified risk factor was older age, then previous DVT.
Management was in accordance with guidelines, with LMWH followed by oral VKA.
Thromboprophylaxis remains underestimated in current recommendations for cancer patients.
These facts justify performing the study in private medical practice.
