Sclérose des grandes veines saphènes à la mousse délivrée par cathéter écho-guidé sur veine vide : alpha-technique bilan des 1.000 premiers traitements

Foam sclerotherapy of saphenous veins through echo-guided catheter into empty vein : alpha-technique results of first 1 000 treatments

Résumé :

Objectif : Faire le point sur les 1.000 premiers membres traités depuis 2001 par scléro-mousse délivrée par cathéter long sur veine vide (ambulatory bloodless catheter delivered endovenous foam) Alpha-technique.
Matériel et méthodes : 1.000 membres inférieurs ont été traités chez 764 patients entre mai 2001 et janvier 2005.
Un bilan écho-Doppler a été systématiquement effectué à la première consultation puis le jour de l'intervention pour un échomarquage et 1 mois après le geste.
Les patients étaient invités à un suivi annuel.
L'intervention avait lieu en ambulatoire, au bloc opératoire et sous anesthésie locale.
Dans 638 cas l'abord s'est fait par phlébotomie distale : jarretière pour la grande veine saphène, tiers inférieur de jambe pour la petite veine saphène. Le cathéter était monté jusqu'à la jonction.
Dans 112 cas une crossectomie de la jonction saphéno-fémorale ou saphéno-poplitée a été effectuée et le cathéter descendu dans le tronc veineux.
Les autres patients étaient porteurs de récidives ou veines résiduelles qui ont été abordées le plus souvent au niveau d'une perforante distale de réentrée.
Après mise en place du cathéter (Royal Flush Cook 5F) une bande d'Esmarch entourait le membre et une compression localisée à la jonction était pratiquée par l'aide opératoire.
La mousse obtenue par méthode du tourbillon de Tessari était à base de tétradécylsulfate de 2001 à 2003 puis de lauromacrogol. Les concentrations du sclérosant de base étaient de 3 % en 2001/2002 et ont diminué ensuite : actuellement elles sont de 1 à 1,5 %.
Le patient se levait de la table d'opération dès la fin du geste pour éliminer tout sclérosant des veines profondes.
Résultats : Tous les axes traités étaient oblitérés au contrôle à 1 mois (928 membres).
5 thromboses veineuses profondes : 3 surales, 2 fémoro-poplitées ont été déplorées, sans complications emboliques pulmonaires cliniques.
Une évacuation de thrombus a été nécessaire sur 15 % des membres en 2002, mais seulement 5% en 2004 après diminution de la concentration de la mousse.
De même, les pigmentations du trajet ont été notées dans 12% des cas en 2002 et dans 3% en 2004.
Une étude comparative à 3 ans a été menée : elle a concerné 52 membres traités par Closure®, 38 par laser endoveineux (Dornier 940 nm) et 60 Alpha-techniques.
Au total, 87% des membres traités par Closure® et 85% des membres traités en scléro-mousse conservaient un excellent résultat hémodynamique et esthétique à 3 ans.
Conclusion : La scléro-mousse par cathéter long permet d'obtenir des résultats très proches de ceux des techniques endoveineuses thermiques pour un prix de revient largement inférieur. Un protocole de surveillance rigoureux dans les semaines et les mois suivants est indispensable pour obtenir cette qualité de résultats.

Summary :

Aim : Review the first 1 000 patients treated since 2001 by the Alpha-technique (foam delivered into empty vein through echo-guided catheter).
Materials and methods : Between May 2001 and January 2005 a total of 1 000 lower limbs in 764 patients were treated. Duplex examination was systematically carried out at the initial consultation, on the day of treatment (for mapping) and 1 month later. The patients were followed-up yearly. This out-patient procedure was performed in an operating suite under local anaesthesia. In 638 patients the catheter was introduced just below the knee for the great saphenous vein and in the lower third of the leg for the small saphenous vein and then passed upwards to the junction. In 112 patients a ligature at the sapheno-femoral/popliteal junction was performed and the catheter passed downwards in the venous trunk. The other patients suffered from recurrent or residual varices which were usually approached through a distal re-entry perforator. Once the catheter (Royal Flush Cook 5F) was in position, an Esmarch bandage was applied, followed by local compression in the junctional area. The foam was obtained by the Tessari technique : tetradecylsulphate was used between 2001 and 2003 and lauromacrogol thereader. The concentrations used were 3% in 2001-2002 and was reduced thereafter, now being 1-1,5 %. The patient walked as soon as the procedure was over to eliminate any sclerosant from the deep veins.
Results : All treated trunks were occluded at 1 month (928 limbs). There were 5 deep vein thromboses (3 in the calf and 2 in the femoropopliteal trunk) without any clinical pulmonary embolism complications. A thrombectomy was necessary in 15 % of limbs in 2002 but in only 5 % in 2004 after reducing the concentration of the foam. Similarly, pigmentation along the course of the vessel was noted in 12 % of patients in 2002 and in 3 % in 2004.
At 3 years, a review was made comparing these results with those obtained in 52 limbs treated by Closure®, 38 treated by endovenous laser (Dornier 940 nm) and 60 Alpha-techniques. Altogether, 87 % of limbs treated with foam had an excellent haemodynamic and cosmetic result at 3 years.
Conclusion : Echo-guided foam sclerotherapy through a long catheter produced results very similar to those obtained with thermal endovenous techniques but at a much lower cost. Such good results can only be obtained by careful follow-up during the weeks and months following treatment.
