But : proposer une dénomination pour les veines perforantes dans un dossier patient partagé.
Contexte : classiquement la dénomination des perforantes était éponyme, variable selon les auteurs et les pays. La tendance actuelle est de classer les perforantes en fonction de leur localisation. Ceci a l'avantage d'être précis mais de créer un nombre considérable de dénominations qui sont trop longues à utiliser en pratique quotidienne.
Un système simple et consensuel manque.
En 1986 sept hauteurs pour un membre inférieur ont été proposées, la cuisse et la jambe étant divisées en tiers anatomiques, la hauteur 1 correspond au tiers supérieur de la cuisse, la hauteur 7 au pied.
En 1999 on associait aux 7 hauteurs les 4 faces anatomiques (latérale, antérieure, médiale, postérieure) pour faire un quadrillage de 28 quadrilatères, repérés par une lettre et un chiffre, par exemple M2 est le tiers moyen de la face médiale de la cuisse, P5 est le tiers moyen de la face postérieure du mollet.
Méthode : les perforantes sont inscrites en fonction de leur localisation dans l'un des 28 quadrilatères (4 faces, 7 hauteurs) dans le dossier patient par une lettre et un chiffre : par exemple M4 ; P6 ; L2 ; A3.
Le système est si nécessaire affiné : perforante excentrée, on précise la localisation dans le rectangle avec la lettre voisine, par exemple M6 P pour perforante en arrière de la grande saphène dite tibiale postérieure ou Cockett.
Enfin les notions classiques peuvent être ajoutées : nombre de cm par rapport à un point fixe comme le sol, le pli poplité, l'interligne articulaire fémoro-poplitée. Avec l'écho-Doppler on note le diamètre en millimètres et l'importance du reflux (absent, minime, majeur).
Exemple d'une dénomination complète : M6 P, 12 cm, 5mm, majeur.
Résultats : seuls les médecins stagiaires en fin d'études adhèrent instantanément au système de quadrillage, les médecins ayant déjà leurs habitudes peinent à en changer.
La simplicité du quadrillage rend la notation très efficace.
Les zones frontières posent parfois problème, par exemple la différence géographique est faible entre M6 P et P6 M. Dans les deux cas le médecin retrouve facilement la perforante nommée mais cette confusion peut poser problème dans le cadre d'études statistiques par rectangle.
Faut-il préciser son caractère court ou long, direct ou non ?
Conclusion : un système simple et consensuel s'impose pour la dénomination des perforantes. Le quadrillage est peut-être une des possibilités à développer. A chacun d'y réfléchir au-delà de ses propres habitudes.

Object : Proposed naming of the perforating veins in the patient file.
Background : The classical naming of the perforating veins is eponymous and varies with authors and countries. The present trend is to classify these veins in terms of their localisation. This has the advantage of being precise but it leads to a multitude of names which are too lengthy for everyday use. Hence the need for a consensus and a simple system. In 1986, seven levels were proposed for the lower limb after dividing the thigh and the leg each into thirds – level 1 corresponding to the upper third of the thigh and level 7 corresponding to the foot. In 1999, 4 anatomical surfaces (lateral, anterior, medial, posterior) were added to these 7 levels resulting in a grid of 28 squares identified by a letter and a number – for example, M2 representing the middle third of the medial surface of the thigh, P5 the middle third of the posterior surface of the calf.
Methodology : In the patient file, the site of each perforator is noted in term of the corresponding square (28 squares : 4 surfaces, 7 levels) defined by a letter and a number – such as, M4, P6, L2, A3. The method can be refined if necessary : an excentric perforator is noted in the corresponding square and the adjoining letter is added, for example, M6 P defines the perforator situated posterior to the great saphenous vein, the so-called posterior tibial or Cockett's. Furthermore, classical methods can be added such as distance in cms from fixed points such as the floor, the popliteal crease, the femorotibial joint line. After duplex scanning, the diameter in mm and the degree of reflux (absent, minimal, major) are noted. A complete identification might thus read : M6 P, 12 cm, 5 mm, major.
Results : Trainee doctors in their final years adopt the grid system immediately while doctors who have their own methods find it difficult to change their habits. The simplicity of the grid system makes identification easy. Frontier zones can sometimes cause problems, for example, the geographical difference between M6 P and P6 M is slight. In both cases the corresponding perforator is easily located by the doctor, but confusion could occur in the context of a square by square statistical analysis. Should further details of the perforating vein be noted such as whether it is long or short, direct or not ?
Conclusion : A simple and consensual method of identifying perforating veins is necessary. Such a method is the grid system which should perhaps be further developed. Personal assesment notwithstanding personal habits is suggested.
